For the information and guidance of all concerned, enclosed is a copy of the Civil Service Commission (CSC) Memorandum Circular No. 11, s. 2017 dated April 6, 2017 mandating the use of the 2017 Revised Personal Data Sheet (PDS) CSC Form No. 212, together with the Work Experience Sheet.

In this connection, all Department of Education (DepEd) officials and employees are required to update their records using the revised form and submit two copies of the accomplished PDS to their respective Human Resource Management Officers (HRMOs).

WHEREAS, after more than ten (10) years of using the PDS 2005 revised form, the Commission recognizes the need to further revise the PDS to conform with the requirements on appointments and other issuances;

In compliance with the said resolution, all employees of the local government of General Santos are required to update their personal data using the new form and submit to the Human Resource Management and Development Office not later than April 30, 2017.

Interested and qualified applicants should signify their interest in writing. Attach the following documents to the application letter and send to the address below not later than Sept. 23, 20221. Fully accomplished Personal Data Sheet (PDS) with recent passport-sized picture (CS Form No. 212, Revised 2017) which can be downloaded at www.csc.gov.ph;2. Performance ratingin the last rating period(if applicable);3. Photocopy of certificate of eligibility/rating/license; and4. Photocopy of Transcript of Records.

Fully accomplished under oath or notarized Personal Data Sheet (CS Form No. 212, Revised 2017) with a recent passport-sized picture (3.5cm x 4.5cm) and Work Experience Sheet (downloadable from www.csc.gov.ph);2.
